The Junior Writer’s Foundation Parent Permission Slip

The Junior Writers Foundation requires that all youth under the age of 18 provide proof of permission from parent or legal guardian prior to participation. The following information needs to be completed and signed and the form returned to your school before the minor will be allowed to participate.

By my signature below, I do release, defend and hold harmless, the Junior Writers Foundation from and against any and all claims, actions, suits or proceedings of any kind or nature arising as a result of the action of this youth.
