The Junior Writers Foundation requires that all youths under the
age of 18 provide written permission from their parent or legal guardian
prior to participation in the Publishing Program. The following information
needs to be completed and signed by your parent or legal guardian
and their permission slip returned to your school before you will
be allowed to participate.
I, the undersigned, the parent/legal guardian of ____________, whom
I declare to be under the age of 18, hereby give permission for my
child, ______________ to participate in the Junior Writers
Foundation Inc.’s Publishing Program.
By my signature below, I do release and agree to hold harmless the
Junior Writers Foundation Inc. from and against any and all claims,
actions, suits or proceedings of any kind or nature arising as a
result of the participation of my child in the Junior Writers Foundation
Inc.’s Publishing Program and agree to reimburse Junior Writers
Foundation Inc. for any court costs and attorneys fees it incurs
in any such suits or proceedings.
